DocConnectCA is a web-based tool that helps high school guidance counselors in assisting Canadian adolescents that are in need of a family doctor. It identifies doctors based on distance and provides print-ready application forms.

Step 1 - You begin by inputting your postal code. The print-ready application form is accessible below the postal code search bar.


Step 2 - Depending on your location, the system will generate a list of doctors nearest to you and provide you with their contact information.

If you want to add family doctors to the database, it is just as quick and as easy as searching for a family doctor.

The World Health Organization has stated that the biggest threat facing adolescents has to do with mental health and wellbeing. Poverty is the single largest determinant of health and poor health is an obstacle to social and economic development. While poverty has many dimensions, poverty and health are inseparably linked. The social determinants – like education, healthcare, and child development – are of great importance to impoverished adolescents. 


The adolescents that are without a designated family doctor in today’s Canada tend to have at least one of two commonalities: they are living in low-income communities and/or they are members of racialized groups. These adolescents face many barriers and the only way they will experience success in life is through education. 


The Canada Health Act ensures that all Canadians have access to healthcare services, but systemic barriers are the reason why we still have Canadian adolescents that are living without a designated family doctor. Having adolescents across Canada that are without a designated family doctor is a matter of national urgency.


The inequitable distribution of health-harmful living conditions are the result of poor social policies and programs, unfair economic arrangements, and bad politics. DocConnectCA has the potential to be a great equalizer and to create generational change, across Canada.


|Our Proposal to the Province of Ontario|

DocConnectCA was created in collaboration with Algonquin College and a joint decision was made that it would be proposed 

as a no-cost solution to the Province of Ontario, focusing on the Ministry of Health and the Ministry of Education. The reports 

below were submitted to the Province of Ontario and other key stakeholders.
